Initial Consultation
This is the first time you and Dr. Peterlin will sit down together to review your full medical chart. Your medical history is vital to obtaining a proper diagnosis and treatment plan. Dr. Peterlin will spend at least 60 minutes talking with you about your family history, past treatments, comfort levels, and diagnostic testing. During this session, you discuss and learn about all of your treatment options and leave with an agreed upon plan. Additionally you will be given instructions on how to submit your bill to your insurance company for reimbursement. Cost for this initial consultation is $645.00 (cost includes same day payment and cash or check discounts).
Most patients will be offered an appointment time within 21 days of our receipt of your completed new patient paperwork!
Follow-up Patient Appointment
Follow up appointments are designed to track your progress and medication management (if you are taking medications). At this time, Dr. Peterlin will talk with you about your progress, discuss how you are feeling, and make recommendations for changes if they are needed. These appointments can take anywhere from 30-60 minutes, depending on your need. We schedule them for 60 minutes since we never want you to feel rushed. You will be given instructions on how to submit your bill to your insurance company for reimbursement. Cost for follow up appointments is $300.00 (cost includes same day payment and cash or check discounts).
Cancellation Policy
We understand circumstances may change. We request patients to notify us more than two (2) business days in advance if they need to cancel or reschedule an appointment. Cancellation or rescheduling requests submitted two or less business days in advance of the appointment will result in a charge of the full visit fee.

Prior Authorization for a Medication: $50 per medication, per year
Discounts Available
Same-Day Payment: If you choose to pay the same day of service, you will receive a 20% discount.
Cash or Check: We do accept debit and credit cards; however, if you choose to pay by cash or check, you will receive a 3% discount.
Family/Friend Referral: Any patient who refers a family member or friend will receive a 20% discount on their next service.

Completing prior authorizations for medications and procedures can be time consuming. These fees help offset the cost and time that our staff spends completing the paperwork for you.
